1. I am the parent/legal guardian of the Applicant. I voluntarily agree to provide the personal information in this Application Form in relation to the YF Life Jr. Space Camp Program organized by YF Life Insurance International Ltd.
2. I certify that the information contained in this Application Form is true and accurate and I understand that any misrepresentation or failure to provide the information in this form may result in the disqualification of the Applicant in the Program.
3. I hereby agree and authorize YF Life Insurance International Ltd. to use the personal data provided in this Application Form and/or photos or videos of the Participants of the Program for the purposes of (a) processing the Applicant's application, (b) arranging the participation of the Participants in all activities of the Program (including but not limited to the participation in the Space Camp) (c) arranging the participation of the Participants in marketing activities in relation to the Program and (d) publishing of marketing and promotional materials in relation to the Program.
4. I hereby authorize YF Life Insurance International Ltd. to transfer or disclose the personal data provided in this Application Form to the media or the Program's service providers or any related organizations or individuals for the above purposes (including both Hong Kong and overseas organizations).
5. I understand that I have the right to access and to correct the personal data provided in this Application Form held by YF Life Insurance International Ltd. by making a request in writing to the Marketing Department at 12/F, 38 Gloucester Road, Wanchai, Hong Kong.
6. I have read and understood the Personal Information Collection Statement and the Points to Note and agree to be bound by their terms.
